Imaging records are available through this easy to use service. Instead of picking up a CD, you can access your records from the comfort of your home.
How Clearpath benefits you
- Your imaging records are stored and are accessible to you at any time, from any device.
- You can instantly share these records with any doctor or family member.
How to get Clearpath
- Simply Create an Account at Myclearpath.com.
- You will be asked to enter in your First and Last Name, Date of Birth, E-mail, and Time zone.
- Once you create an account you will get a temporary password sent to your e-mail
- Use the temporary password and your email address to log in
- Request your records
How to request records
- Once logged into Clearpath, select the ‘Request Record’ tab.
- Select the ‘Request Record’ tab.
- To request a record, select the ‘Request Record’ button on the bottom right corner.
- Select ‘Record Custodian’ (Facility Location)
*If you do not see the facility name listed, please reach out to help@myclearpath.com . - Select which Records to Request, Body Part, and Date Range.
- Agree to the terms and conditions and submit.
*Once submitted, we will review and process. Once request is processed, if approved, you will see your images under the ‘My Records’ Tab.
